The Ohio Home Care Waiver is designed to serve frail, disabled adults between 18 and 59 years of age, who have functional limitations, and who have been referred by a physician or other medical authority.

Eligibility Criterea:

Client must be:

• Between 18 and 59 years of age
  
• Living in their own home or apartment
  
• Frail enough to require hands-on assistance with the activities of daily living
  
• In agreement with their physician and provider regarding an approved plan of care

Services Provided:

• Personal care
  
• Respite
  
• Skilled nursing
  
• Physical/occupational/speech therapy
  
• Medical social services
